乳腺实性乳头状癌的病理学分析

摘要

探讨实性乳头状癌(solid papillary carcinoma,SPC)的病理学特征、免疫组织化学及相关的鉴别诊断。收集5例SPC患者的资料,进行病理特征观察,并进行免疫组织化学染色检测ER,PR,Her-2,Syn,CgA,Ki-67,p53,CK5/6,p63,CD10,calponin,并复习相关文献。5例患者中,仅1例因肿瘤破裂不适就诊,其余均无明显乳头溢液。镜下改变:均可见实性结节状结构,并见纤维血管轴心,呈乳头状生长,其中2例伴有黏液分泌,其中1例合并有黏液癌。免疫组织化学:ER,PR高表达4例,超过80%,Her-2均阴性。Syn不同程度阳性3例,CgA阳性1例。Ki-67<5%4例。p53在所有患者中<5%。SPC是一种有独特病理特征的乳头状肿瘤,好发于老年女性,属低度恶性,发病率低,预后较好。

Pathological analysis of solid papillary carcinoma of breast

Abstract

To investigate the pathological features, immunohistochemistry and differential diagnosis of solid papillary carcinoma (SPC). The data of 5 patients with SPC were collected, and their pathological features were observed, and the immunohistochemical staining for ER, PR, Her-2, Syn, CgA, Ki-67, p53, CK5/6, p63, CD10 and calponin were retrospectively analyzed. Relevant literature was reviewed. Of the 5 cases, only 1 case was diagnosed because of tumor rupture and discomfort. There was no obvious nipple discharge in all cases. Solid nodular structure and papillary structure were seen under the microscope in all cases. Mucous secretion was seen in two cases, there was one case with mucinous carcinoma. Immunohistochemistry showed that ER and PR were strongly positive, more than 80 percent, in 4 cases, and Her-2 was negative in all patients. Syn was positive in 3 cases, CgA was positive in 1 case. Ki-67 <5% in 4 patients. p53 expression was less than 5% in all patients. SPC is a kind of papillary tumor with unique pathological features, which usually occurs in elderly women with low malignancy, low incidence, and good prognosis.
