目的：观察地塞米松穴位注射治疗外耳道湿疹的疗效，并分析其安全性。方法：选择医院2017年9月至2018年9月收治的符合标准的外耳道湿疹患者90例，按1:1:1比例将患者分为对照组(A组和B组)与治疗组，每组各30例，对照组A组采用可的松软膏局部治疗，对照组B组采用氯雷他定片治疗，治疗组采用地塞米松穴位注射治疗，治疗14 d，比较3组患者治疗总好转率、相关症状消失时间、瘙痒视觉模拟评分(Visual Analogue Scale，VAS)评分以及不良反应发生情况，并统计随访6个月3组患者复发情况。结果：治疗组的治疗总好转率(90.00%)明显高于对照组A组(60.00%)、B组(63.33%)，组间差异有统计学意义(P<0.05)；治疗组湿疹、红肿、瘙痒等症状消失时间明显短于对照组A组、B组，组间差异有统计学意义(P<0.05)；治疗后1周、1个月3组瘙痒VAS评分明显低于治疗前(P<0.05)，治疗组明显低于对照组A组、B组，组间差异有统计学意义(P<0.05)；6个月后，治疗组的治疗复发率(3.33%)明显低于对照组A组(26.67%)、B组(23.33%)，组间差异有统计学意义(P<0.05)；治疗14 d，治疗组1例青年男性患者出现糠秕孢子菌毛囊炎，对照组A组出现2例嗜睡、头痛，B组出现2例嗜睡、恶心呕吐，均不影响继续治疗，组间差异不存在统计学意义(χ2=0.424，P=0.809)。结论：地塞米松穴位注射治疗外耳道湿疹疗效明确，安全可靠。
Efficacy of acupoint injection of dexamethasone in the treatment of eczema of external auditory canal

Objective: To observe the efficacy of acupoint injection of dexamethasone in the treatment of eczema of external auditory canal, and analyze its safety. Methods: A total of 90 patients with eczema of external auditory canal who met the criteria and were admitted to the hospital between September 2017 and September 2018 were included. The patients were divided into control group (group A and group B) and treatment group according to a 1:1:1 ratio, with 30 cases in each group. Control group A was given local treatment of cortisone ointment, and control group B was treated with loratadine tablets, and treatment group was given acupoint injection of dexamethasone, and they were treated for 14 days. The overall improvement rate of treatment, disappearance times of related symptoms, pruritus Visual Analogue Scale (VAS) score and occurrence of adverse reactions were compared among the three groups, and the recurrence was statistically analyzed in the three groups at 6 months of follow-up. Results: The overall improvement rate of treatment group (90.00%) was significantly higher than that of control group A (60.00%) and group B (63.33%) (P<0.05). The disappearance times of symptoms of eczema, redness and pruritus in treatment group were significantly shorter than those in control group A and group B (P<0.05). The pruritus VAS scores in the three groups at 1 week and 1 month after treatment were significantly lower than those before treatment (P<0.05), and the score in treatment group was significantly lower than that in control groups A and B (P<0.05). After 6 months, the recurrence rate of treatment group (3.33%) was significantly lower than that of control group A (26.67%) and group B (23.33%) (P<0.05). After 14 days of treatment, 1 young male patient in treatment group had pityrosporum folliculitis, and 2 patients had drowsiness and headache in control group A, and 2 patients had drowsiness and nausea and vomiting in group B, and the symptoms did not affect the continuation of treatment (χ2=0.424, P=0.809). Conclusion: Acupoint injection of dexamethasone has exact efficacy in the treatment of eczema of external auditory canal, and it is safe and reliable.
