肌肉骨骼超声联合股前外侧穿支皮瓣移植术在四肢创面修复中的应用

摘要

目的:研究肌肉骨骼超声联合削薄股前外侧穿支皮瓣(anterolateral thigh perforators flaps,ALTP)移植术在四肢创面修复中的应用。方法:采取前瞻性研究,选取2017年2月至2020年1月在无锡市第九人民医院进行治疗的四肢创面修复患者120例为研究对象,随机分为观察组与对照组,每组60例。观察组采取肌肉骨骼超声联合ALTP移植术进行治疗,对照组只进行ALTP移植术治疗。比较两组的治疗效果、围手术期指标、并发症之间的差异。结果:观察组的手术时间(t=20.941,P<0.001)、术中出血量(t=58.588,P<0.001)、住院时间(t=2.556,P=0.012)显著低于对照组,两组的皮瓣切取面积的差异无统计学意义(P>0.05);观察组(95.00%)的治疗优良率显著高于对照组(81.67%),差异有统计学意义(χ2=5.182,P=0.023);两组供区变更、皮瓣并发症、供区并发症的差异均无统计学意义(P>0.05)。结论:四肢创面修复中采用肌肉骨骼超声联合ALTP移植术治疗的效果显著,建议临床推广。

Application of musculoskeletal ultrasound combined with anterolateral thigh perforator flap transplantation in limb wound repair

Abstract

Objective: To study the application of musculoskeletal ultrasound combined with anterolateral thigh perforator flap (ALTP) transplantation in limb wound repair. Methods: In this prospective study, 120 patients with limb wound repair who were treated in our hospital from February 2017 to January 2020 were selected as research objects. According to the principle of random grouping, the above patients were randomly divided into an observation group and a control group, with 60 cases in each group. The observation group was treated with musculoskeletal ultrasound combined with ALTP transplantation, while the control group was only treated with ALTP transplantation. The treatment effect, perioperative indicators, and complications were compared between the two groups. Results: The operation time (t=20.941, P<0.001), intraoperative blood loss (t=58.588, P<0.001) and hospitalization time (t=2.556, P=0.012) of the observation group were significantly lower than those of the control group, and there was no significant difference in the flap area between the two groups (P>0.05). The excellence and good rate of the observation group (95.00%) was significantly higher than that of the control group (81.67%), and the difference was statistically significant. There was no significant difference in donor site change, flap complications, and donor site complications between the two groups (P>0.05). Conclusion: In the treatment of limb wound repair, musculoskeletal ultrasound combined with ALTP transplantation is effective, and it is recommended to be popularized in clinic.
