目的：观察五行音乐疗法改善慢性乙型肝炎(chronic hepatitis B，CHB)(肝郁脾虚证)患者焦虑、抑郁症状的临床效果。方法：选择2019年1月至2020年6月海安市中医院收治的CHB患者76例，随机分为对照组与观察组，每组各38例。对照组给予护肝、降酶、抗病毒等常规治疗，同时给予健康宣教、情志安抚；观察组在对照组常规治疗的基础上，采用中医五行音乐疗法，7 d为1个的疗程，连续治疗3个疗程。比较患者焦虑、抑郁、中医证候积分的变化，检测肝功能指标谷丙转氨酶(alanine aminotransferase，ALT)水平。结果：治疗1个月、3个月、6个月与对照组相比，观察组患者的焦虑自评量表(Self-Rating Anxiety Scale，SAS)评分均显著降低，差异有统计学意义(P<0.05)；抑郁自评量表(Self-Rating Depression Scale，SDS)评分亦显著降低(P<0.05)；中医证候积分均显著降低(P<0.05)；两组患者ALT均好转，观察组患者的ALT水平相对较低，但差异无统计学意义(P>0.05)。结论：五行音乐疗法利于改善CHB患者的焦虑、抑郁情绪，减轻临床症状。
Effects of five-element music therapy on anxiety and depression in patients with chronic hepatitis B (liver depression and spleen deficiency syndrome)

Objective: To observe the clinical effect of five-element music therapy on the improvement of anxiety and depression in patients with chronic hepatitis B (CHB) (liver depression and spleen deficiency syndrome). Methods: A total of 76 patients with CHB admitted to our hospital from January 2019 to June 2020 were randomly divided into a control group and an observation group, 38 cases in each group. The patients in the control group were given routine treatments such as liver protection, enzyme lowering and antiviral therapy, as well as health education and emotional comfort. The patients in the observation group were given five-element music therapy that derives from traditional Chinese medicine on the basis of the treatment routine in the control group. A course of treatment was 7 d, and the treatment lasted for 3 courses. Scores of anxiety, depression and TCM syndrome score were compared, and the level of alanine aminotransferase (ALT) was detected. Results: After 1 month, 3 months and 6 months of treatment, the SAS scores of patients in the observation group were significantly lower than those in the control group. The differences were statistically significant (P<0.05). The SDS scores were also significantly decreased (P<0.05). The TCM syndrome scores of the observation group were significantly decreased (P<0.05). The liver function index ALT was improved in both groups, and the level of ALT in the observation group was relatively low, but the difference was not statistically significant (P>0.05). Conclusion: Five-element music therapy is beneficial to improve the anxiety and depression symptoms of patients with CHB (liver depression and spleen deficiency syndrome), and reduce the clinical symptoms of patients.
