IL-37联合CA125、HE4对上皮性卵巢癌的诊断价值

摘要

目的:探讨白细胞介素-37(interleukin-37,IL-37)联合糖类抗原125(carbohydrate antigen 125,CA125)、人附睾蛋白4(human epididymis protein 4,HE4)对上皮性卵巢癌(epithelial ovarian cancer,EOC)的诊断价值。方法:选取2018年1月至2020年12月在南京市中西医结合医院经术后病理确诊的76例EOC患者(EOC组)和卵巢良性肿瘤患者80例(良性组),均行血清IL-37、CA125、HE4水平的检测,比较各组指标水平差异,并绘制受试者工作特征(receiver operating characteristic,ROC)曲线评估各指标单独及联合对EOC的诊断效能。结果:EOC组血清IL-37及CA125、HE4水平均高于良性组,差异有统计学意义(P<0.05)。血清IL-37水平与EOC患者肿瘤大小、FIGO分期、有无淋巴结转移存在相关性(P<0.05),而与年龄、绝经状态无明显相关性(P>0.05)。ROC曲线分析显示,IL-37诊断EOC的曲线下面积为0.737(95%CI:0.659~0.805),CA125对应的曲线下面积为0.857(95%CI:0.791~0.909),HE对应的曲线下面积为0.746(95%CI:0.642~0.808);三者联合的曲线下面积为0.932(95%CI:0.879~0.966),高于所有单一指标,差异有统计学意义(P<0.05);且三者联合诊断早期EOC的曲线下面积为0.952(95%CI:0.706~0.893),也高于单一IL-37[0.867(95%CI:0.769~0.934)]、CA125[0.822(95%CI:0.717~0.900)]、HE4[0.812(95%CI:0.706~0.893)],差异均有统计学意义(均P<0.05)。结论:血清IL-37与EOC肿瘤大小、临床分期及淋巴结转移相关,对EOC具有一定诊断价值,联合CA125、HE4可提高诊断的效能。

Diagnostic value of IL-37 combined with CA125 and HE4 in epithelial ovarian cancer

Abstract

Objective: To investigate the diagnostic value of interleukin-37 (IL-37), combined with carbohydrate antigen 125 (CA125) and human epididymis protein 4 (HE4) in epithelial ovarian cancer (EOC). Methods: Seventy-six patients with EOC (EOC group) and 80 patients with benign ovarian tumor (benign group) pathologically diagnosed after surgery in our hospital from January 2018 to December 2020 were selected. The serum levels of IL-37, CA125 and HE4 were detected. The differences of each index level were compared between each group, and the receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic efficacy of each index and its combination for EOC. Results: The levels of IL-37, CA125 and HE4 in the EOC group were higher than those in the benign group (P<0.05). The level of IL-37 was correlated with tumor size, FIGO stage and lymph node metastasis (P<0.05), but not related to age and menopausal status (P>0.05). ROC curve analysis showed that the area under the curve of IL-37 diagnosing EOC was 0.737 (95%CI: 0.659–0.805), the area under curve of CA125 was 0.857 (95%CI: 0.791–0.909), and the area under curve of HE was 0.746 (95%CI: 0.642–0.808), and the area under curve of the combination of the three was 0.932 (95%CI: 0.879–0.966), which was higher than the single index, and the difference was statistically significant (P<0.05). The area under the curve of the three combined diagnosis of early EOC was 0.952 (95%CI: 0.706–0.893), which was also higher than that of single IL-37 [0.867 (95%CI: 0.769–0.934)], CA125 [0.822 (95%CI: 0.717–0.900)], HE4 [0.812 (95%CI: 0.706–0.893)], the difference was statistically significant (P<0.05). Conclusion: Serum IL-37 is associated with tumor size, clinical stage and lymph node metastasis of EOC, and has certain diagnostic value for EOC, combination of CA125 and HE4 can improve the specificity of diagnosis.
