电针联合综合康复治疗膝骨性关节炎对患者疼痛、关节功能及关节液中TGF-β1水平的影响

摘要

目的:探究电针联合综合康复治疗膝骨性关节炎(knee osteoarthritis,KOA)的疗效及对患者疼 痛、关节功能、关节液中转化生长因子-β1(transforming growth factor-beta1,TGF-β1)水平的影 响。方法:选取2020年1月至2021年3月海口市中医医院收治的110例KOA患者为研究对象,按患 者治疗意愿分为常规组(n=58)与电针组(n=52)。常规组予以综合康复治疗,电针组在常规组基础 上增加电针治疗。比较两组临床疗效及中医证候积分、疼痛[疼痛视觉模拟评分(Visual Analogue Scale,VAS)]、关节功能[Lysholm膝关节评分(Lysholm Knee Score Scale,LKSS)]、关节液中 TGF-β1和炎症因子[白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor- alpha,TNF-α)]水平。结果:电针组临床总有效率明显高于常规组(90.38% vs 75.86%,P<0.05)。 电针组治疗后中医证候积分显著低于常规组(P<0.05)。与常规组相比,电针组在治疗14 d、治疗 30 d时和随访时的VAS评分均更低,LKSS评分均更高(P<0.05)。电针组治疗后关节液中TGF-β1水 平较常规组更高,IL-6、TNF-α水平则更低(P<0.05)。结论:电针联合综合康复治疗KOA疗效显 著,可明显改善患者临床症状,缓解疼痛,增强关节功能。提高TGF-β1水平、抑制炎症反应可能 是其作用机制。

Effect of electroacupuncture combined with comprehensive rehabilitation on pain, joint function and TGF-β1 level of joint fluid in patients with knee osteoarthritis

Abstract

Objective: To explore the efficacy of electroacupuncture combined with comprehensive rehabilitation in the treatment of knee osteoarthritis (KOA) and its influence on pain, joint function, and the level of transforming growth factor-β1 (TGF-β1) in joint fluid. Methods: A total of 110 KOA patients admitted to our hospital from January 2020 to March 2021 were selected and divided into a control group (n=58) and an observation group (n=52) according to different intervention methods. The control group was treated with comprehensive rehabilitation, and the observation group was treated with electroacupuncture on the basis of the control group. The clinical efficacy and TCM syndrome score, pain [Visual Analogue Scale (VAS)], joint function [Lysholm Knee Score Scale (LKSS), joint fluid TGF-β1 and inflammatory factors interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α)] levels were compared between the 2 groups. Results: The total effective rate of the observation group was significantly higher than that of the control group (90.38% vs 75.86%, P<0.05). The TCM syndrome score in the observation group after treatment was significantly lower than that in the control group (P<0.05). Compared with the control group, the VAS scores at 14 d, 30 d and follow-up in the observation group were lower, and the LKSS scores were higher (P<0.05). After treatment, the levels of TGF-β1 in the synovial fluid of the observation group were higher than those of the control group, while the levels of IL-6 and TNF-α were lower (P<0.05). Conclusion: Electroacupuncture combined with comprehensive rehabilitation has a significant effect on KOA, which can significantly improve the clinical symptoms of patients, relieve pain, enhance joint function, increase TGF-β1 level and inhibit inflammatory response may be its mechanism.
